Forms and Paperwork

For all students completing community engagement projects within a course, please fill out the following forms:

1. Community Engagement Student Proposal Form: This form is must be completed by the end of week eight.
   
   
2. Consent Form: Please print out the form, sign and turn it in to your professor.

3. End of Semester: Community Engagement Student Form: This form must be completed by the end of finals week. Students completing action research should still submit this form as well.
